Submission Instructions
In your submission, please highlight how your project or program meets one or more of the Evaluation Criteria developed by the National Dialog Steering Committee, and how it provides an outstanding example of the application of one or more of the CSS Core Principles.
Submissions are welcome from transportation agencies, planning agencies, communities and other organizations. Submissions will be accepted through July 31, 2009.
Step 1: Select and Open the Form
- On the Submission Forms page, select the category your submission fits into and select the form for that category.
- A .pdf form will open in your browser. You must have either Adobe Acrobat or Adobe Reader. Adobe Reader is available as a free download from http://get.adobe.com/reader/?promoid=BUIGO.
- If you are uncertain about which form to use, you may wish to review the forms before you begin or contact us at cssnationaldialog@ncsu.edu or 919-515-9351.
- If your project, plan or program seems to fit in more than one category, choose the category that best fits; the form is flexible enough to accept explanatory detail on multiple facets of a submission.
Step 2: Save the Form
- Save the form to your local computer. You must have Adobe Acrobat or version 7 or higher of the free Adobe Reader in order to save the form. You may update your version of Adobe Reader using the link above.
Step 3: Complete the Form
- Complete the sections of the form that apply to your submission. If more than one person will fill out the form, it may be saved to your computer and sent to others as an e-mail attachment.
- You can tab through the form fields or click in the field you wish to complete.
- Checkboxes. Form checkboxes can be checked by using the spacebar, the enter key, or by clicking on them. A second click will remove the check.
- Textboxes. To enter text in a text box, either tab to or click inside the box and type directly into the form. You may also copy and paste text from another file (for example, from a Word document).
- Text boxes will expand to fit the amount of text entered. Some boxes may shift to the next page to accommodate entered text. Depending on the amount of text entered, you may have blank areas between the fields in your completed form.
- If a question does not apply to your submission, simply enter "NA" in the text box.
Step 4: Attach Any Supporting Documentation
- Pictures, plan documents, design guidelines, video files and other supporting documentation are welcome.
- Please list, by filename or document title, any supporting documentation you are submitting in the text box at the end of the form.
- Attach these documents to the form submission e-mail.
Step 5: Submit
- Click the "Submit by Email" button at the top of the form. This will generate an addressed e-mail message with a subject that will send a file containing your responses.
- If you are using an Internet-based e-mail client such as gmail, etc., you will need to save the form to your computer and send it as an attachment to cssnationaldialog@ncsu.edu.
